A friend of mine had Morton's neruma also so bad she couldn't wear dress shoes. She went to a sports medicine orthopedic doctor and had surgery which helped tremendously. Also you could contct the Orthopedics department in a major university medical center. You may have to travel but this is usually where you will find the best doctors. You can call the department of Orthopedics and ask the secretaries if any of the orthopods have any particular interest in Morton's neuroma. If you have trouble doing this ask your internsist for a referral, he or she can make the inquiry for you but it should be easy enough for you to do. Sometimes the secretaries know the best doctors. I know because I was one of them!
