Have you tried getting your records from the hospital where the operation was performed? It seems like the information you want SHOULD BE there. But, if not, you may be able to glean the names of those present during the time of your operation from your file, or who are familiar with what was done, who are still here and can answer your questions (nurses, operating room attendants, etc.?) Barring that you could try to track down the doctor. You might find a helpful soul among his associates if they know you're not trying to complain, you just have a question. If he has sold his practice, perhaps his successor has a forwarding address? If all else fails, it seems like he could be found through the internet search services once he is re-established.
