I, too, have suffered from chronic hives for almost a year and a half and I have been on 4 of the same meds as you. I still have not found a doctor who can find a cure for my hives, nontheless a reason. Right now I am taking hydroxyzine at night, which honestly isn't helping, plus, cetaphil cream with menthol. The cream does help the itching and burning but it takes a while to take effect. I finally got tired of the doctors in my area and am going to a University hospital to seek answers. All I can offer you right now is good luck and tell you to hang in there. I know it's VERY frustrating, but, without hope, we have nothing.
