My son had this problem when he was 3. We took him to notied child gastroenterologist in Philadelphia. The treatment was mineral oil. We started with a big dose and tapered until the bowel movements were no longer painful. He did have a re-occurance and we had to start the mineral oil again. It was successful. He is now 17 and no longer has problems.
