Piriformis muscle syndrome is frequently -- if not almost always -- a symptom of Sacroiliac Joint Dysfunction. My guess would be that when you were hit by the vehicle, your SI joint was moved out of alignment, thus causing other muscles, like the piriformis, to take on compensatory behaviors. This leads to chronic spasm. You can treat the piriformis muscle with physical therapy -- ultrasound, massage and release techniques -- but if the underlying problem is SIJD, you won't experience resolution of the problem. The worst-case scenario, which I experienced, is that, left untreated, eventually the pelvic floor muscles will also be pulled into chronic spasm, and you could experience the most agonizing pain imaginable. You need to be evaluated by a knowledgeable D.O. or very experienced physical therapist with specific training in SIJD. Good luck!
