My 3 year behaves like Jeckyl & Hyde

My 3 y/o girl is quiet & pleasant at the babysitters. When we get home (she is usually hungry) she will scream for 1 hr befor we can get her consoled and fed. At bedtime, she will sometimes make a 1 to 2 hr display. In the morning, she ALWAYS begins screaming at the moment her eyes open and it ALWAYS continues for at least 30 minutes. The triggers for tantrums are innumerable, but our problem is twofold: we cannot seem to prevent them and we cannot seem to stop them once they begin. There are some unique elements to her episodes. ( We have tried everything from loving her to ignoring her...) While inconsolable, it seems as if she cannot even hear or understand. She rambles about strange things. She sometimes exhibits violence. Her speach and physical progression is somewhat ahead of her chronological age. Trips away from home seem to distract her from these activities. If she is sick, tired or hungry you can count on an episode. She can be so sweet. Lastly, I believe that she really tries to behave, at times, and fails and becomes irrational and inconsolable. This really frustrates her. I'm afraid shes sick but I live in a really small town with no decent Psych for kids.
