Have you been checked for Sjogren's Syndrome which also has a speckled ANA? Do you have dry eyes and/or dry mouth along with your other symptoms? Go online and look up speckled pattern ANA. You should find the various things that can have the pattern you have. Then look up the individual diseases to see which symptoms match yours best. You can then ask the doctor to test you for that disease. (I had a whole lot of trouble getting a diagnosis of Sjogren's because it affects everyone differently.)
